HOW TO CHANGE THE NAME OR CORRECT MISTAKES ON YOUR PASSPORT

STEP 1: COLLECT DOCUMENTS

A. Download, COMPLETE, AND SIGN Passport Application (DS-5504)

IMPORTANT NOTE: This link will take you off site to complete the form.  Complete form online, when you are finished, look near the bottom of the form, you will see a "Create" button, create the form, then "Print" the form.  Once printed close that window to return to our site.  Please print two copies.

B. Submit Two recent and identical passport type or size photographs

C. Submit current valid U.S. passport (photocopies are not acceptable)

Be certain your passport is not excessively worn or damaged, If so, you may want to apply for a first time passport.

D. Certified copy of legal document that caused the name change, e.g. a marriage certificate, divorce decree or court order.

If you are using this service to correct your passport which may have been recently issued with errors such as name or date of birth, you must submit either a certified copy of your Birth Certificate or previously issued passport bearing the same credentials. Copies are not acceptable.

E. Download AND COMPLETE Letter of Authorization

F. submit evidence of your departure date (ie air itinerary or copy of ticket)

G. Download AND COMPLETE Order Form

STEP 2: PREPARE PAYMENT

A. FIGURE YOUR FEES USING OUR FEE CALCULATOR

Normally, there is no fee for this service. You just mail in the required documents directly to the U.S. State Department (The address is on the form). However, this process will take 4-6 weeks. In choosing to use International Passport Visas, Inc. to expedite your passport, you must then pay an expedite fee to the U.S. government. They add this fee to any service they provide. Normally it is free; when expedited they charge $60.00. This fee is more like a penalty fee, and is in addition to the fee that we charge. Our fees are for services we provide when standing in line at a Federal Passport Agency for many hours. This is the most assured way to guarantee the fastest possible issuance of your United States Passport.

STEP 3: MAIL YOUR DOCUMENTS TO US
A. MAIL YOUR DOCUMENTS VIA FEDEX

1) Place the following documents in a FedEx Priority Letter:
  • Valid or current United States Passport
  • Application form DS-5504 completed and signed
  • Letter of Authorization
  • Certified Copies of Documents causing name change
  • Proof of departure date (itinerary or copy of ticket)
  • Completed Order form
  • Payment to International Passport Visas, Inc. (see step 2)
2) Ship Overnight Services.

We recommend that you use FedEx as a safe means of overnight shipping. We will return your passport to you the same way.

IMPORTANT NOTE: You are sending this package to a FedEx facility for our service to receive the package to submit to the US Passport Agency by 10am. This ensures the fastest possible service; however, the FedEx facility will deny packages sent via other overnight services (UPS, Airborne, Express Mail, etc) - therefore it is imperative you send using FedEx.
