STEP 1: COLLECT AND FILL OUT DOCUMENTS

A. Collect all the necessary documentation listed under New Passports

B. Collect all the necessary documentation listed under Passport Renewal

C. Complete and sign TWO Separate Letter of Authorizations

D. OBTAIN A Letter from THE Employer

The letter should be addressed to the United States Department of State requesting the second passport be granted to their employee explaining the need for such a request on 8.5" x 11" paper (signed).

D. OBTAIN A Letter from the applicant

The letter should be addressed to the United States Department of State requesting the second passport, explaining the need for such a request on 8.5" x 11" paper (signed).

E. Download, complete, and sign State Department form requesting second or additional passport.

F. submit evidence of your departure date (ie air itinerary or copy of ticket).

G. Download AND COMPLETE Order Form

STEP 2: PREPARE PAYMENT

A. FIGURE YOUR FEES USING OUR FEE CALCULATOR

STEP 3: GO TO YOUR LOCAL PASSPORT ACCEPTANCE AGENCY

A. Have your local Acceptance Agency Witness your DS-11 form.

Be sure to familiarize the clerk with your need for a 2nd or Additional US passport. The clerk will go through the following process:

  • To verify your identity, the agent will request your proof of identity.
  • The agent will ask you to sign the DS-11's, and then sign and seal it themselves.
  • To verify that you need expedited passport services, you will need to show them your travel documentation.
  • The agent will then seal the DS-11's, photos, proof of citizenship, and all other requirements and related Fees in an envelope marked "To be Opened by U.S. Passport Office Only".

  • IMPORTANT NOTE: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".


B. Do not open this sealed envelope or the application will be denied.

C. Please write the FOLLOWING INFORMATION on the outside of the sealed envelope:

    1) Applicant Name
    2) Date of Birth
    3) Departure Date

     

STEP 4: MAIL YOUR DOCUMENTS TO US
A. MAIL YOUR DOCUMENTS VIA FEDEX

1) Place the following documents in a FedEx Priority Letter:
  • Sealed envelope containing your DS-11 and all accompanying documentation
  • DS-82 form
  • Two recently taken and identical passport type or size photographs
  • Authorization Letter
  • Request letter from your employer (see step 1)
  • Request letter from you
  • Completed Order form
  • Payment to International Passport Visas, Inc. (see step 2)
  • Copies of seven documents
2) Ship Overnight Services.

We recommend that you use FedEx as a safe means of overnight shipping. We will return your passport to you the same way.

IMPORTANT NOTE: You are sending this package to a FedEx facility for our service to receive the package to submit to the US Passport Agency by 10am. This ensures the fastest possible service; however, the FedEx facility will deny packages sent via other overnight services (UPS, Airborne, Express Mail, etc) - therefore it is imperative you send using FedEx.
