48 PAGE PASSPORT
Be sure to complete the form DS-4085 "Request for Additional Passport Pages" along with whatever other request form you are using, e.g. DS-11 or DS-82 - thus indicating that you want the additional pages added to your passport at the same time the new or renewal passport is being issued.  Click here for NEW PASSPORT or here for RENEWAL PASSPORT requests and be sure to complete the DS-4085 Additional Pages form at the same time.  It is also recommended that you attach the decal below in the upper right hand corner of your passport application, drawing additional attention to your request.
