CHILDREN'S PASSPORT

Choose this Service if:

  • The Passport Applicant is under the age of 16 (15 years of age or less), and
  • The Child applicant is a United States Citizen by birth, or Naturalization or has an Assignment of Citizenship by as U.S. Court or Magistracy, and
  • You are applying with this applicant as (parent, listed on the Child's Certified copy of Birth Record) court ordered guardian or custodian of this Child Applicant.

STEP 1: COLLECT AND FILL OUT DOCUMENTS

A. COMPLETE ONLINE PASSPORT APPLICATION (DS-11)

IMPORTANT NOTE: This link will take you off site to complete the form.  Complete form online, when you are finished, look near the bottom of the form, you will see a "Create" button, create the form, then "Print" the form.  Once printed close that window to return to our site.  Please print two copies.

B. Provide Proof of U.S. Citizenship

    Examples:
  • Certified copy of Birth Record
  • Previous U.S. Passport
  • Certificate of Citizenship or Naturalization

C. Proof of Identity for Both Parents

Each parent or guardian must submit one of the following:
  • Valid Drivers License
  • Valid Official U.S. Military ID
  • Valid U.S. Government ID
  • Valid U.S. or Foreign Passport with recognizable photo
  • Naturalization/Citizenship Certificate from BCIS with recognizable photo
  • Alien Resident Card from BCIS.

D. Submit Two recent and identical passport type or size photographs

E. Submit current valid U.S. passport (photocopies are not acceptable)

Be certain your passport is not excessively worn or damaged, If so, you may want to apply for a first time passport.

F. Provide Social Security Number of Child.

If you do not provide the Social Security Number the IRS may impose a $500.00 penalty fee.

G. Download AND COMPLETE Letter of Authorization

H. submit evidence of your departure date (ie air itinerary or copy of ticket).

I. Provide evidence of the Child's Relationship to the Parents or Guardian

You must submit one of the following:

  • Child's certified birth certificate (with parents' names)
  • Child's certified foreign birth certificate (with parents' names and translation if necessary)
  • Child's report of birth abroad (form FS-240 with parents' names)
  • Child's certification of birth abroad (form DS-1350 with parents' names)
  • Adoption decree (with adopting parents' names)
  • Court order establishing custody
  • Court order establishing guardianship

If the parent(s)/guardian(s) name(s) is/are other than the names on the documents, evidence of legal name change is required. Previous U.S. passportsare not acceptable as proof of relationship to the applying parent(s)/guardian(s).

J. Both parents must be present with the child at the acceptance agent in step 2, or one parent must be present with a notarized Form DS-3053 and the child present.

STEP 2: PREPARE PAYMENT

STEP 3: GO TO YOUR LOCAL PASSPORT ACCEPTANCE AGENCY

A. Have your local Acceptance Agency Witness your CHILD's DS-11 form

In order to apply for an expedited U.S. passport you must first have your Childs Application form witnessed by an acceptable authority. Normally this is done by certain Post Offices and County Clerks. For a listing of the closest Acceptance Agent to you click here. Both parents must be present ALONG with your child during this process. If one of the parents cannot be present a form DS-3053 must be completed by the absentee parent and NOTARIZED.

B. Before going for witnessing to the acceptance agent, be sure to make copies of the following documents, then take the original documents with you:

  1. Application Form DS -11
  2. Proof of Citizenship
  3. Proof of Identity for both parents
  4. Two Passport Photographs
  5. Passport Processing Authorization Letter
  6. Proof of Departure Date
  7. Fees

C. Go to a the Passport Acceptance Facility with all documents mentioned above.

Most facilities are open M-F, 9am to 4pm. Notify the agent you are using an expediting service to "hand carry" your application.

The acceptance agent will take about ten minutes to go through the following steps:

  • To verify your Childs identity, the agent will request your proof of Parentage. (Both parent's must be present with Child, or one parent with notarized Form DS-3053 and Child present).
  • The agent will ask BOTH parents to sign the DS-11, or the agent will ask for absentee form DS-3053 and ONE parent to sign the DS-11. He/She will witness your signature and seal ALL documents in an envelope for Hand-Carrying.
  • To verify that you need an expedited passport, you must provide evidence of your departure date.
  • The agent will then seal the DS-11, photos, proof of citizenship, letter of authorization, proof of departure date, and the check for the government fees in an envelope marked "To be opened by U.S. Passport Office Only".

  • IMPORTANT NOTE: Make sure the agent returns the sealed envelope to you. If the agent does not feel at ease about releasing the envelope, suggest that he/she refer to page 31 or 44 of their Passport Agent's Manual, "Hand-carrying of Executed Applications".

D. DO NOT SEAL THE ORDER FORM OR PAYMENT FOR OUR SERVICES (INTERNATIONAL PASSPORT VISAS, INC.) INSIDE THE SEALED ENVELOPE.

E. Do not open thE sealed envelope or the application will be denied.

E. Please write the FOLLOWING INFORMATION on the outside of the sealed envelope:

    1) Applicant Name
    2) Date of Birth
    3) Departure Date

STEP 4: MAIL YOUR DOCUMENTS TO US
A. MAIL YOUR DOCUMENTS VIA FEDEX

1) Place the following documents in a FedEx Priority Letter:
  • Sealed Government envelope
  • Copies of seven Documents
  • Completed Order form
  • Payment to International Passport Visas, Inc. (see step 2)
2) Ship Overnight Services.

We recommend that you use FedEx as a safe means of overnight shipping. We will return your passport to you the same way.

IMPORTANT NOTE: You are sending this package to a FedEx facility for our service to receive the package to submit to the US Passport Agency by 10am. This ensures the fastest possible service; however, the FedEx facility will deny packages sent via other overnight services (UPS, Airborne, Express Mail, etc) - therefore it is imperative you send using FedEx.
